No no, It was resolved but it came back.  Im going to keep an eye on it and let you know.  My mods are deleting them when they see dupe posts.

It's definitely the the redirect type and mod_rewrite combination. Make sure the redirect type is set to "HTML Redirect (If others fail)" setting. That fixed it for me and hephnet, who were both experiencing the same problem.

Increase your flood limit, and see if that helps? It could be an impatient user, on a slow connection hitting the register button to quick, but doing a flood of 30 seconds or even one minute, may help
