Because of some complications we've had, a community that I am a part of is moving to another forum. We're searching for the theme from our old one, but we've had no luck. Here's the forum:

Thats a heavily modified custom theme, did your community modify it themselves? because you won't find that in any of the eblah themes... :O

If you can't get that old one back, or have a backup, I'd say its lost my friend

If you still have access to the files on your server, you can copy the theme from the templates directory in blahdocs. Then re-upload it to your new server.
