A lot of people have asked for this, though in different forms.  I've had it for awhile, but just haven't taken the time to post it.  I hope it helps.

Basically, it adds extra boxes to the registration proceedure and to the Name and Profile information in the Member Center.  The options are for "Full Name" and then address information "City," "State," "Zip" (sorry for those of you not in the US!  I originally made this for my own use!)  There are also 4 user defined fields that can be anything you want.  Each field can be set as required or optional.
